Why This Form Is Important

• It is a sworn legal declaration. The deponent affirms the contents on oath, and the verification states that the contents are true and correct to the best of the deponent's knowledge and belief.
• It identifies where the error appears. The deponent names the certificate, the relationship of its holder to the deponent (for example self or my son), and the name that was wrongly or incompletely mentioned in it.
• It puts the correct name on record. The deponent states what the correct name is, so there is a written statement of the name as it should read.
• It declares that both names belong to one person. The deponent affirms that the correct name and the wrong or incomplete name are both the deponent's names and the names of the one and the same person, which is the core purpose of the affidavit.
• It records citizenship and residence. The deponent states being a citizen of India and presently residing at the stated address.
• It closes with an affirmation of truth. The deponent states the above statement is true and correct, which the verification block then repeats.
