Affidavit declaring the correct patient particulars and requesting correction of inaccurate or inconsistent information in hospital records for medical, administrative, or official purposes.
Why This Form Is Important
It is a sworn statement. The deponent "solemnly affirm[s] and declare[s]" each clause and signs as DEPONENT twice (after the clauses and after the verification), so every statement is a personal, signed declaration. A false sworn statement can carry legal consequences (general knowledge of Indian law, not taken from an official policy document).
It identifies the person making the request. The opening line records the deponent's name, father's or husband's name and residential address, and clause 1 states that the deponent is a citizen of India.
It shows the deponent's connection to the patient. Clause 2 records the deponent's relationship with the patient and names the patient and the hospital in New Delhi where the patient was admitted, so the reader can see who is asking for the correction and which hospital record it concerns.
It lists exactly what is wrong and what is right. Clause 3 sets out, side by side, the wrong and correct patient name, patient's husband's name and address, so the hospital or receiving authority can correct each detail without guessing.
It puts the correction on record as the deponent's own statement. Clause 4 and the verification state that the contents are true and correct to the best of the deponent's knowledge and belief, with the date, month and year of verification recorded at New Delhi.
General note: the form does not say what the corrected record will be used for or which authority will receive it. Beyond the points above, the importance of a correction affidavit of this kind rests on general knowledge, not on an official policy document.
